Austin Health is the major provider of tertiary health services, health professional education and research across 3 campuses in the northeast of Melbourne and state-wide. Austin Health is world-renowned for its research and specialist work in cancer, liver transplantation, spinal cord injuries, neurology, endocrinology, mental health and rehabilitation.

An exciting opportunity currently exists for an enthusiastic and highly experienced Grade 2 Dietitian to join our Diabetes Care Management and Assessment Service (DCAS) and Wound Clinic, Health Independence Program (HIP).

The Austin HIP services aim to assist people with chronic disease, aged and/or complex needs that frequently use hospitals or at risk of hospitalisation. The HIP services offer a client-centred and integrated delivery of services and are primarily located at the Health and Rehabilitation Centre which is the 'one stop shop' for the these programs.

The Dietitian will ensure that the nutrition and dietary requirements of clients referred to the Diabetes Service and Wound Clinic are met through assessment, provision of appropriate diet, monitoring, discharge planning and education for patients and carers.

The Dietitian/s employed in these positions will be based at the Austin Heidelberg Repatriation Campus. The hours of work are rostered on a Thursday morning (4 hours) in the Wound Clinic and on a Friday morning (4 hours) in the Diabetes Service.

To be successful in this role you must demonstrate:

Minimum of 2 years post graduate clinical experience working with clients with chronic diseases and associated needs, including an understanding of the community services available and associated referral criteria.

Sound clinical nutrition and dietetics knowledge in a variety of areas with demonstrated depth of knowledge in area of clinical speciality.

Demonstrated ability to provide leadership, and to work effectively both autonomously and within a team environment.

Demonstrated high level communication skills, effective counselling skills and a commitment to patient-centred care.

Demonstrated ability to establish realistic and achievable goals and identify priorities and measureable outcomes.

Demonstrated ability to show initiative, flexibility and to problem solve in a variety of complex situations.

Evidence of involvement in quality improvement activities related to dietetic care.
